Permit Application Forms
The forms listed below are necessary for an individual to apply for a Permit for radiation sources or radiation emitting equipment.
A Radiation Worker Registration form must be included for all individuals who will be handling radioactive materials or working with radiation emitting sources or equipment.
Every application for a Permit must contain an Application to Use Radiation Producing Sources. Depending on the scope of the research project or program, additional forms for radioactive materials, specific sealed sources, or irradiators can be attached.

Inventory Forms
A series of inventory forms have been developed. Forms for receipt and disposal of unsealed radioactive materials are computer generated when a purchase order is processed by the Radiation Safety Staff, and transmitted to the Permit Holder or facility LRPO.
Sealed Source Inventory Record and Radiation Emitting Equipment Inventory Record forms are used to document the receipt and possession of contained sources and other devices.

Personnel Dosimetry Forms
A series of personnel dosimetry forms were developed to assist consignees for dosimetry accounts in managing that program.

Bioassay Worksheets
Individuals who use quantities of hydrogen-3 or iodine-125 in excess of certain limits must be monitored for uptake of these radioisotopes. The use of other radioisotopes such as >30 mCi of carbon-14 and >10 mCi of phosphorus-32 may also require a bioassay. Two worksheets have been developed to document the bioassay.

Instrument Efficiency Worksheets
As an aid to Permit Holders working with radioactive iodine-125, two worksheets have been developed to enable them to calibrate survey and counting equipment for low levels of activity. These worksheets document instrument performance required for regulatory compliance.
The Radiation Safety Staff maintains NIST traceable standards and phantoms to assist Permit Holders in instrument calibration.

Several standardized worksheets have been developed to allow Permit Holders a uniform method to report the results of surveys required by the Radiation Safety Program.
